ALL PATIENTS WILL BE REQUESTED TO SIGN A CONSENT FORM ACKNOWLEDGING OUR PRIVACY PRACTICES PRIOR TO TREATMENT. You have the right not to sign this consent form. However, this would necessitate the physician to refuse treatment.

  • Request corrections to your medical records
